Well a lot of why people who get into diapers appear to sprout from what I'm going to call fetish theory. You see when you're growing up especially at young impressionable ages and when you go into puberty, random things from the "normal" to "abnormal" can suddenly become rather fascinating. Using males and breasts for example, maybe you were breast fed as a child, and when you were entering the earliest stages of puberty maybe you saw someone with large breasts and it made you happy. The happiness makes you associate it with positive, and you try to see if the same or similar experience makes you happy again. Several successful positive experiences can make it become very associated with pleasure and can even cause arousal by that stimulus to form. Thus as a guy becomes mature and sees breasts he might get a boner from it. Now we can use this same theory to apply it to diapers. As a baby maybe a guy felt the most safe and most comfortable when swaddled in diapers and may have had a lot of very personal parent child moments happen during things that involve diapers such as getting a change when it's making you feel really bad to be in a soiled one. Those positive experiences become subconsciously locked in. Then later on going into those age 10-12 years maybe he gets a sibling and it's cute to see them running around in diapers. Potentially even sparking some curiocity of what it'd be like to wear them again and makes him daydream about it. Further positive experiences. It could even be something as random as a positive experience from meeting someone incontinent for the first time and enjoying how the garment made them look. Eventually once again the positive experiences make the garment be associated with pleasure, and can potentially develop into a fetish. Then from there it just goes into an evolving road that can go from trying the garment for the first time, to becoming completely enveloped by the fetish lifestyle and community and it becoming a part of you. Even there some don't go as far as others, such as many not liking diapers unless they're clean. So ultimately the diaper fetish springs likely from the fetish theory of how all fetishes can develop from positive experiences during impressionable times in ones life. (Also thank the media so many cartoons work in stuff like diapers, inflation, even the ever obvious furries. Very impressionable stuff)

We can thank the psychologists who put all their time and effort into trying to figure this out for that. I just hope someday humans have the time to get a deeper understanding of all these things as we're still pretty in the dark despite what we already know. We know why people like things like boobs or butts, but as for things like this and more, we really are still learning and we just have to go with what theories we have.
