I didn't really go into it before, but her master/mentor/teacher/whatever-you-want-to-call-it was General Bane, which is a character I came up with.
As for the door, this chapter takes place a little while after the first, around the time Allen and Lenalee are in the town where time rewinds and they first meet Road. the door was referring to the ones Road creates
