I don't quite remember, but I think I used a blood brush, made it flatter, then warped it to get the curve, then used bevel and emboss to make it look more 3d.

I supose its up to you it would be safe to give the site but not absolutly nessisary. As long as people know you put some work into this and its not simply a screen shot ripped straight from a game i think your fine.
