So to clarify, that sword IS her horn. Cyan has a dark portal in her forehead where the sword is summoned from, plus you can see the design of the hilt is the same. I'm not sure why Abyss used the version without the portal in her forehead as the default version, but you can see what the portal is meant to look like in the alts in the description.

The portal only stores the 1 sword.
The idea is Cyan was abducted by cultists who used her body as a medium for a summoning ritual. They dried to open a portal to summon their slumbering demon lord to come to their plane and awaken. However the ritual was interrupted, and only part of the demon’s sword came through.
She feels no pain from pulling the sword in or out because the portal is a literal hole in her head. Her skin and everything are missing/on a different plane of existence.
