Seems like he's being hit? I see something behind him that seems like it's going to break his back and a "splash" behind him that looks like blood. Perhaps the "wheel" in the title refers to the wheel of time, and how one day, he's going to die too, even though he currently looks young and healthy? Or maybe he's experiencing some kind of trauma that is the result of something "bad" he did in the past, and the wheel is referring to some kind of karmic force.

I have not so many words about because it is a bit hard to explain for me too and I 'don't want to' find an explanation. I want that you all could feel it as you want to! ^^
