Quite a masterful use of negative space. There's so little to see but, for some reason, I don't have any problem with it. The harsh, but scarce, light and the stark contrast pair nicely with the deliberate gaunt, sunken eyes and reinforced distress of the mouth. It's THE expression for a piece like this.
