So here's my Dr. Whooves and Assistant theory... in the future, before Dinky comes in the picture, Derpy and the Dr. have two little colts while traveling time and space, then suddenly, one of the Dr's enemies steals the boys and takes them back in time and leaves them. The Dr. and Derpy search everywhere, but they can't find them. That's why, when Derpy found out she was expecting again, the Dr. took her home to care for Dinky alone, while he looked for the boys, never knowing that their past loyal companion was one of their long lost sons. It could explain why Derpy keeps Dinky's father a secret (so the enemy couldn't find and take her too), and how Tic Tock was able to create the things he did, he had seen them before, and was, in fact, recreating them from very early memory. It may also explain why the Dr. became discorded and hated Derpy, she was a constant reminder of his failure, of how he couldn't save the two little boys he loved more than the world. Truth be told, he never hated Derpy, he hated himself and wanted her to hate him too, but when she refused to, he resorted to trying to force her too, which only made her want to help him all the more. He still searches for the boys, and vows to himself to never stop until he has and can bring them home.
