I can't share the Cydia icon since it's from a NFR theme, but there's a similar icon from ComeBack 2. Check the thread on MTF. But the lockscreen widget is Word, [link] and lockscreen theme is Cirq, [link]

It's originally a BarLCD, but for this setup, i've added the Translucent_Base/BarLCD on the wall.
