Iirc, Madoka's wish in the timeline removed all witches.
Nagisa never became a witch because Madoka's wish essentially was a giant cosmic retcon. Charlotte is Nagisa's witch form, Bebe is an in-between.

They said that the one who made it is not the official maker of MSMM because of the lack of the story and some of the events are kinda weird and confusing like the wraiths has suddenly vanished , Charlotte has become Nagisa ( that mostly fans never expected that )

Momoe is officially the human form of Charlotte, as of the third movie.
In the anime they didn't give her a human form because they did not expect such a popularity of her from fans.
