there's a pretty headcanon/theory-based backstory but!! the simple part is that Paradox is a human shell (the scientist who got sent to time hell and promptly died) inhabited by, and acting pretty independently of, a time-based eldritch abomination that's sleeping in time hell. that's where he got his time powers and ability to breach different universes came from.
that picture is specifically when he gets his arm lopped off, and some eldritch blood is seeping out since the shell is broken... he's crawling towards Azmuth for their "first meeting" to get a replacement arm
