Seems normally you blend out the shadow, but in both SAI tests you've left it solid so I've been noticing the odd spaces. It could be just me though.

No idea, as I'm not really sure what your drawing method is. The way I do it it'd be basically impossible for stuff like that to happen, so I can't really help you there.
Eh, just keep an eye out for that in the future, I guess. Then you'll probably be able to figure out what happened.

Oh, and one other tip, if you're drawing freehand on a non-linework layer, I recommend using the line stabilizers. I usually keep mine around level 10 but since you're a mouse user you might want to go with a stronger one. Those clouds look a bit shaky.
