This is our AU (run by 5 people) called Calamitytale, a post-pacifist AU where OC's come into the mix. I have a comic you can view in my gallery that explains the story better than I can in words, but to answer your other questions the pony you see here is the villain (sort of like Chara mixed with Flowey/Asriel) and she is about to battle with Papyrus. The image is in spoiler territory since the comic isn't finished yet so i can't go into detail there.

Ok. Basically put the pony is (or was) an imaginary friend of the main character who is also a pony. The main pony character replaced Frisk and fell down and freed the monsters. In this post-pacifist AU the demon pony returns after the main pony has an argument with Mettaton and proceeds to kill everyone.
