Actually women in power and high positions, specially in military roles, are often called Sirs as a sign of respect as Ma'am is generally found to be degrading. This is because of gender roles and feminism, women in power want to be called Sirs because Ma'am is considered weak and they want to be treated like men superiors (i.e. called the same name and shown the same respect)
This seems to mostly happen in movies or tv shows though, not sure about real life, so I'm not sure if the show actually had Shizune call Tsunade that or if the translators were being influenced by how Hollywood handles this. Also the show might have used some unisex word and the translators just went with what they knew.
Aaaaaaanyway, apart from all that this pic made me laugh XD Lets just say I hit a lot of branches when I fell out of the boob tree and people have "Still" confused me with a guy!

screwed up translations welcome to the american version of most anime where words only slightly follow the actions and complete blocks of episodes are cut out to make everything PC aimed at younger groups then originally intended in japan yay us
