It's heavily implied that the Epilogue is given by the Didact after the events of the game implying that he survived the destruction of the Mantle's Approach and the 30mt nuclear detonation. He's somewhere out there. Also, knowing 343I, they wouldn't toss off such a convenient villain. They aren't that creative.
