I usually browse www.davidrumsey.com/ for inspiration regarding how to frame the map. After that, I make a rough sketch in QGis. QGis also helps me georeference any kind of basemap and reproject them into any kind of map projection, to make things easier.
The rest of the work is done in Adobe Illustrator.
