This piece is very, good in several ways. I'll try to cover everything I can think of but in all honesty I don't think I could touch on everything even if I spent several hours at it.
First and foremost, I like how you've deviated from the actual GF art style and how carefully you've done so. Knowingly or not this picture is noticeably more detailed/realistic than the cartoon in a way that conveys a lot of tension and weight while sticking close enough to retain the 'feel' of the show, staying true to the spirit of the original is how I'd read it.
I've got a few notes for why I think this; the large size of Stan's ears, the line of his mouth, the anatomically incorrect way his eyebrows are behaving and the texture of his nose are all very authentic while the extra details like a cracked spectacle lens and the veins/creases on his hands make it a bit more grounded and immediate. A few other touches like the detail of his shoes and the blood (which someone else mentioned) really do cement this feeling without making it seem overbearing.
The lighting, fog and all the other background elements frame Stan beautifully as well as pushing the dread of the situation centre stage. Even if I hadn't seen Scary-oke I'd have a damn good idea what sort of situation he's in and they work nicely with his expression and posture to give a nuanced strength to the picture. I think this was far, far more powerful than, say, an equally detailed replication of the actual background he would be standing against in the show.
The use of colour and reflection in the suit, shoes and knuckedusters bridge the gap between 'cartoon character' and 'old man in a dire situation'. Battle damage alone wouldn't have done it, I think, and overall these with everything I've already mentioned set up an interesting contrast between his slightly comical appearance and the intensity of the stare and the upper half of the background (I have no idea what those coloured streaks are meant to be, if anything, but they make me feel unsettled and tense).
I have one or two minor criticisms, although these could easily be explained by stylistic choice. I *believe* Stan is one of the five-fingers-per-hand crowd so seeing him without pinky fingers confused me for a second. His face seems to be lit from beneath while almost everything else is lit from above and behind. Looking at his trousers I'd instinctively say that he has his right foot forward at least a little, but his feet are clearly level with each other.
These criticisms are all very nitpicky and possibly personal only to me, but I figured I wouldn't be doing you any favours by ignoring them.
All that said I'd happily buy a print of this for £20-30 and consider it a good deal... shame he's a TM'd character really. Kudos!
(Final note, I rated this in what might be seen as an odd way. 2 stars is 'good but nothing special', so if you think of 3 as a middle of the road score you should probably bump every category up by half a star)

Attention to detail is amazing! The expression, shading and fine details like clothes are wonderful.
The brass knuckles actually look shiny and solid, wouldn't want to be on the receiving end of those!
The zombie blood and Stan's own is great to see, (not in a sadistic way) The face you put it there and that it's not only on his face but on or under his clothing is a great detail to put it.
The background is simple, dark and eye catching.
It's a good amount of eye catching too, not so eye catching it draws you away from Stan but just enough to set the mood.
All in all it looks fantastic!

Vision: This hypes me!!! It was an excelent episode Scary-oke. But this picture captures the esence of the season's beginning. Stanford Pines as a real badass, that kick asses for the twins...
Originality: I have seen too many drawings on the internet related to Grunkle Stan and the golden knuckles. Nevertheless, it's the first time that Stanford gets too highlighted on a picture. It's normal that all the GF fans artists drew Grunkle Stan after the episode, because his entrance to the climax was just epic...
Technique: Marvelous work highlighting Grunke Stan. I have just saw a few flaws, but the reflections of the glasses and the knuckles, besides the thorn clothes... This details are pure gold!!!
Impact: This is the most epic picture of Grunkle Stan, related to Scary-oke episode, ever!!!
