Well, you search for commissioners (cosplay.com's forum should be helpful), then yontact them vio PN or mail and talk about what you want and how much it will be. They most likely will make you an offer, you pay for it, they sew whatever you want with your measurements and send it to you.
I don't know how they prefer getting payment. I guess most people will accept paypal ^^

Well, I got the wig some time ago from ebay. It already had a nice shade, but I decided to color it again and so I added some orange to it ^^
