This is a really intense image. it has a lot going for it, including pose, expression, shading and shadow, lighting choices, and overall concept. Much to like and appreciate.

A couple of concerns:
1. It's not immediately obvious how her wrists are attached to the lower bar, nor why she should be making fists (except for dealing with overall pain).
2. If she's been there that long, her face ought to be more "drawn" – that is, showing signs of exhaustion, especially with her tongue pierced and weighted. Needs tears, and/or drool, and/or forehead creases, and/or eyebrow curves, and/or more pronounced bags under her eyes, etc.
3. Her thighs don't seem to be long enough, even given the perspective and POV you've chosen. If we're supposed to assume that most of her legs are pulled behind her, then we should be seeing the soles of her feet somewhere, and her torso ought to be canted a bit forward. But if she's supposed to be kneeling on that bottom bar, then she needs more length in her thighs.
