Rename all the icons, according to which app you want them to (com.android.mms = messaging) etc.
Create a description XML file. Zip all the icons into icons.zip. Take off the .zip and it's a 'file.'
Zip the description xml with the icons file. Name it whatever you want, then take off the .zip and make it .mtz.

Well you have to download his theme and unzip it. Includes icons, page indicators, wallpaper, etc.
