Not quite sure how he symbolizes gluttony. If he was at least fat, maybe it would make some sense.

Gluttony doesn't necessarily refer to the consumption of food and drink. It's rather the doing of things to needless excess.
I think the design fits based on the cloud drifting from his mouth. He's consumed so much that there is an "afterbreath" of sorts. Lol. But that's me.
