Uhhmmmm .u. This is Benedict Cumberbatch. The actor who plays Sherlock in BBC Sherlock XDI don't know what to say here???
Uhh. This took about 3-4 hours to finish, and the last 2 of those hours were done over livestream XD

This is a really lovely portrait that portrays the character well!
Firstly, the choice of the black and white colour scheme is effective since it brings out an "old fashioned" and thoughtful atmosphere that definitely links to the character.
I think that overall, you've done really well to be aware of all the lighting and reflections, but I think you could add even more realism to this by increasing the sharpness of some areas, it almost seems a little too blurry throughout. I would recommend you start with some really sharp white highlights inside the eyes, and maybe some throughout the wrinkles in the forehead too. The lips may also benefit from some further distinction by this method.
Next, I'd like to suggest something I've heard mentioned by digital portrait painters, the idea of introducing skin imperfections with a custom brush, using the scattering option (that is if you use photoshop, I am not experienced in others). Even just set to a very low opacity it helps to reduce the likeliness of the skin coming out plastic or otherwise flat.
I think that most of, adding some more grainyness to this picture will help to bring it more alive, things are well placed and thought out, but if you want to achieve more realism, zoom in more, and work at that level, even if it's things you think will be so subtle that they'll hardly be noticed. If working from a reference picture, place it alongside, zoom into both and look back and forth from one to the other as you work, all the time, always looking for really small things you can emulate in your own picture. Even photos in general can reveal things that you can include to gain more realism.
To finish, I love the composition and idea of this, and I expect you've already learned an awful lot from making this picture. I encourage you to continue working on pictures like this, adding more and more detail as you go! This one has a lot of personality and impact.

The way the portrait has Sherlock look straight ahead at the viewer is striking. ^^ I really like this! Again, I favor the playing of shadows. This adds an over all denisty to the character as well as drawing the viewers eyes straight at the expression (because everything else is in darkness except for the most important features of his face). Also, I like how the light is coming out from the darkness onto his face translating into how it can be viewed this way with this theme: "light can always come out of the darkness." ^^
