You did well with the lighting and shaders, they give out a very great disturbing feeling (since this picture is meant to be disturbing... right?). Although, I think it'd be better to have that arm holding the knife let down, or the knife being at another perspective...

And yeah. I tried to fix the skirt so it wouldn't show her panties, but it was really difficult. I tried out different camera angles that wouldn't show them, but nothing really worked :s
