it's very sad to think about but it seems like not only was he smileing to show sasuke that he still loved him but also because he knew that his own pain would soon end. very well done it realy captures the emotion that the scene carried.

I think the sadest about that smile is, that it actually tells us, that Itachi in the end was happy, it was over.
BTW, I don't think there are many People who could take a life of so much misfortune and sad decesions without going insane.
