Sasuke kills Sakura.The phrase YOU CAN'T UNDERSTAND ME is the same that these 2 characters are thinking about: Sasuke can't understand Sakura's feelings and Sakura can't understand Sasuke's hate.
When I was drawing this picture, I had a specific song in my mind:
