IMHO, the head shape looks a little too human (i.e round), and I think the ears and the horn [on his nose] should be bigger. Otherwise great work, awesome detail!

Don't listen to ~c69-is-me. It's because of the lack of aggression and humanization of the character that this is one of the best. Not everybody has to be in your face and angry all the time. This is a sleeping giant that keeps his destructive tendencies under wraps until you cross him, then he let's loose. It can be terrifying. And the humanization only makes you feel sorry for him. The mutation is a crime against nature, and you can almost see what he used to be. It's great.
