The makeup but the composition of the photo is somewhat messy, and the overall mood tells me that this is straight through artificial. You could easily rearrange it a bit, and possibly tweak the colours and tones to make it look more realistic.
On the other hand, there are loads of "realistic" bloody photos here at dA, so it's kinda a nice break too...
