I think, and I'm not sure here, but I think it has to do the last caption block of the last page.
From what I can tell, it looks like all the plans and work Catherine has put into making David into Delilah has been ruined. My guess is David knows what is happening, is lying his butt off and is throwing his pills away. I just cant figure out the epic level of estrogen he has in his system...... unless and this is a huge leap of guessing.... No one knows it but the truth is, David/Delilah is a hermaphrodite. Just a wild guess and it can't be justified at this point.
