I think this was before the show writers made it prominent as a nod to the fans.

I'm pretty sure her eyes went un-straight prominently around... I'd say halfway in the first season since I know I atleast saw them derped in episode 15 Feeling Pinkie Keen.

This wall-eyed pony (not in this screencap, but others) was first seen in the first episode, as one of the background ponies during Pinkie Pie's surprise party. It was either an animation error or one of the animators was just messing around. One particular sharp-eyed brony over at /co/ in 4chan posted a picture of it and named her Derpy Hooves. A huge fanbase erupted soon after, overwhelming the show's staff with much love for the strange looking pony.
Since then, Lauren Faust and the directors have specifically requested that the pony's model, originally just a generic background character, be given wall-eyes as a nod to the fans. Lauren has even sketched Derpy and it was sold for over $3,000 dollars.
I personally love her, and I love playing "Where's wall-eye" in each episode.
Lauren/other animators have announced that Derpy Hooves will be canon in the second season, with her own model sheet and preset expressions.
TL;DR: Derpy Hooves, originally just a weird lookin' pony, has become a canon fan character.
