This is a very good portrait of Pinhead, you went in the right direction to go black and white with this because I feel as though color wouldn't add much in the way of it being horrifying.
Everything else about this stands out pretty well, the shading on Pinhead's face is well taken into consideration. Especially around the eyes where they don't look visible and that enhances the macabre feel to the art itself, the smoke coming from the back is very well done. How was that done by the way because it looks like it must have taken a while to really make the smoke stand out clearly and add that into the image.
Overall though this is very good work.
