I believe it is an Acaste andersoni. Please see below:

I think you should be able to find it (this illustration) from the article. I think that is the only(?) whole phacop that I did for publication.

Trilobites are my favourite palaeosoic invertebrates.
And we have some interesting localities of sylurian and devonian trilobites fauna in Poland so your A. andersoni is an interesting visitor here
