The thing on the right is conceived as a set of gaping wounds through which the rhino fires balls of cancerous tissue filled with pus which are used instead of smoke launchers for obscuring line of vision. So, in game-terms, it is a smoke launcher.

That peeling effect on the front is very unique, and I like the three eyes on the far right as well.

The peeling thing is actually a thin layer of green stuff, and the "eyes" are actually open pustules ready to spew necrotic tissue and pus, acting like smoke launchers.
