Well there are two different memes.
The ones on deviantart that look like this are just blank files that ask questions and require you to draw and/or write out the answers (Some being deviations, some journal entries).
Ones that are almost everywhere else are Internet-famous concepts that go viral (become famous). So like an image that people find funny and is spread around.
