How do you decide whether something should be black and white or in colour? It seems like such an enormous decision to me!

It really is! I usually try pictures in black and white when they have a really nice texture or interesting lines/composition, sometimes this stands out more when there are no distracting colours. And sometimes to be honest I just don‘t like the colours of a photo and want to see whether it looks good in monochrome
