Here's your answer: Do it yourself and see how hard it is to do.
It's a different answer for everyone. Different amounts of time practicing, different programs used (and the time learning to use said programs).
You also have to know how to draw, first and foremost. Everything starts as a sketch, you have to have a firm grasp on anatomy (which is not the same as realistic anatomy), and even further still a sense of color coordination.
If I may be so bold, using stock images is leaps and bounds easier than drawing something like this, even for beginners. Beginning artists struggle horribly at first with hand-drawn things. Just sort by newest and you'll see what I mean.
