What program do you use? Piskel has been working ok for me but there is no brush variety or rotating items, and I need a better one.

i use paint tool sai though its not free ;-; though you can trial for 30 days

Do you make all the frames then save them then combine them on a different program?

depends on the icon.
Frame by frame like this:
Save each frame indivdually and draw each frame individually in sai and load it into layers in photoshop im pretty sure its script > load files into stack
then create animation > make animation from layers
then save for web and make sure its below 15kb.
or
easier is gifmaker.me but i tend to loose quality and the file size is often larger (and sometimes my animations are a little too involved for like 5 frame limit i get with gifmaker.me

I save each limb, wing, movement, hair pieces, head, horns, accessories on a separate transparent file. (ill draw them all together but on different layers in sai ) so ill end up with usually 5-11 images.
then load these into a library in Adobe Flash CS6 in actionscript 3.0
make a new (Layer? im not sure what its called in flash but its on the timeline. ) for each limb piece in order from bottom to top, so usually like tail> back legs, front legs, body, head, hair, etc.
Then animate all these individually using tweens and a few frame by frame images.
then export> movie > images > png (so its transparent)
then i will open all the frames back in sai to make sure everything is right and fix up anything thats kinda gone wacky and resave then repeat the photoshop thing to save as a gif.
it seems like a lot of effort but the second method is actually a lot quicker.
