It's a combo of photo elements, photo textures and just subtle painting. subtle is always the key with cloth.

Makes sense, I should give that a shot. My concern is that I want to be in control of the design, how do you drive the painting without letting the the photo elements rule your creative decisions?

Use them, bend them to your will, don't let them dictate what your wanting to do. PS has lots of great tools for manipulating photographic elements.
