this is a wonderful photograph! that's what I'll start off with, but there are a few things I'd like to see improved (you don't need to follow my advice at all)
The eyes should be in focus (I know this is hard, especially as a self portrait, but try using autofocus and a mixture of other things)
I really like the depth, the difference in the focus and such, it's really nice to look at and definitely adds to the idea of the photograph, however, the lighting would really improve the shot if it is from straight ahead (as this looks like a studio shot I don't think this should be too difficult)
all in all I really like this shot, the idea works and it makes the model look really handsome.

Hi, thanks for ask.. it is done by camera... you must have right lens woth low apriture.. and have it wie open and that is all.. that will make you small field(deep) of focus

Thanks, I tried it with my f/1.8 lens and I got something that resembled the effect I your picture
