It's not awful, just simple.
The item in her (his?) hand looks like a cowerie shell, a special kind of seashell. I believe in Japan, they symbolize old memories, but I could be mistaken.

From what I can see, the little thing she's holding is shaped like a heart. Just a guess, taking into account the title of the picture. I see that the red thing on her chest/stomach is cracked too. Hm...
