It's overused to the point where it has no effect anymore. There was a time when few people used it. Now too many people use it and it's boring and repetitive. A lot of the time when people use it they over do it too. It's meant to be subtle and have a point to it. Gonna reuse an example: If a character is going insane the artist could mess up his hair to add to the fact that this guy is going mental.

Also, someone started this and everyone is just taking the piss.
