He's actually based more on Aztec and Mayan warriors, kings, priests, and gods. He's Quetzalcoatl's son.

Yeah. There's a story in the tie-in comics about how Kotal Kahn traveled to Earthrealm centuries ago and was worshipped by the Aztecs as a god, and he taught him the Osh Tek's method of gaining power by eating the hearts and drinking the blood of the fallen. And then he went back home to Outworld and his dad Kotal Ketz is like "What the fuck! You know those guys weren't Osh Teks, right?" And then Kotal's like "Oh shit" and he runs to Earthrealm and all his followers are dead from blood poisoning and the Spanish are just stealing everything they can carry.

Well you have to admit, they had a very powerful empire. Did you know that it wasn't Montezuma that feared the Conquistadors, Spaniard explorers? In fact, he wasn't even alive when they arrived. He was actually Montezuma the second, the more incompetent ruler of Aztec. Basically, they were like the ancient Assyrians. They wanted to expand their lands with military force, but was met by destruction from within. From rebellions, to united forces, and by Aztecs standpoint, disease.
