I'm a bit late with the critique but for this particular piece if we're wanting to stray away from "innocent" and more towards manic/nuts I'd furrow the eyebrows, and as Kleineroma said twist the grin some more into a sadistic sneer, widen the eye whites, maybe diminish the pupils.

Try adjusting the grin to be a bit more sadistic. Think something along the lines of The Joker. Also maybe if you widened the eyes a little bit it would help.
