this is my fan made kamen rider unlimited henshin list...(sorry, there are still only five form, the others will come soon)
form right to left
black : mystique mantis
white : platinum centipede
red : red ant
green : steam caterpillar (can turn into butterfly)
blue : machine stag beetle

From left to right:
G3-X head with Faiz Blaster body and thighs and... looks like Blade's legs with DecaDriver.
Kabuto Masked torso and helmet with RioTrooper visor and G3 legs with DecaDriver.
Bat Imagin with Den-O Liner Form body and Joker Undead visor with DecaDriver.
Joker Undead antenna with Ryuutaros head, a unknown kaijin body and legs.
Saga legs and shoulders, Dark Kiva torso, with Tora Claw arms, and an odd head with DecaDriver.

Blue form :
G3-x antenna, psyga helmet, Faiz Blaster body accel trial form hand - leg...
Green form :
Kabuto Masked helmet, RioTrooper eye den-o gunform body shoulders I made it by my self (combine from the bee shoulder and gun form sholder) leg ixa, & kabuto boots...
Red form :
helmet I made it by my self, torso kabuto hyper form, shoulder den-o sword form, leg and hand den-o liner form, leg made it by my self, boots decade....
Platinum form :
Joker Undead antenna with Ryuutaros eye and mouth bat imagine head, body ryuutaros claw joker, shoulder momotaros, leg made it by my self, boots ryuutaros...
Black form :
sasword head, antenna made it by my self, torso saga, hand chalice with tora claws, shoulders ryukendo gold, legs garo cape, and kiva emperor legs...
hahahahahaha, that was the secret...
