Arico and Naruto didnt know that they siblings (sister and brother) until Chuunin exam (Forest of death).They were divided after birth. As for her surname, so no one knew they were relatives, it's difficult say or she was absent or was different.

You know I usually don't like Narutos sister OCs, mostly because Naruto wouldn't be the same person he's in the canon, if he had a sister(or a brother or an aunt etc.), but you kind of make it some what plausible by having them meet during Chuunin Exam. That way Naruto still grows up alone and becomes the person he's in the canon, but having Arico be his twin sister allows her to exist and it actually makes sense to separate them from each other(or just that Arico is adopted as a baby, because she's cute and not a Jinchuuriki, while Naruto is never adopted), since not being with Naruto allows Arico more normal childhood.
The design is very cute and I really love the outfit, though she might look bit too much alike with her brother(though often opposite gender twins do look alike in fiction, so maybe it's just that). Her not meeting Naruto during their time at the Academy makes sense, since she would be told to stay away from him, and honestly at that age both boys and girls prefer to play with kids of same gender mostly(because opposite gender is viewed as "gross").

I like seeing some of the more difficult types of Ocs being done plausibly. Aricos backstory works, since it doesn't really contradict canon(other than her being Narutos twin).
