Shade... you can't see what she is watching from this perspective but most likely she's using her magic orb to watch people have sex. This couple must be kind of boring to watch though otherwise I'd imagine she'd be a little more interested. Some mages are careful about how they use these orbs and then there's Shade who uses it like a television... Also yes the red skull is a soft pillow. She made it herself by taking the skull of one of her victims and pillowizing it.

You have to forgive her though it's because she gets bored easily.
