Description The 19 implanted organs are very complicated, and because several of them only work properly or at all in the presence of other implants, the removal, mutation or failure of one organ can affect the precise functioning of the others. Many would be recruits don't survive the inhumanly painful and involved process all the way to the end. 

The most Distinctive of these implants is the black carapace; an unsightly machination, once applied, the carapace grows invasive tendrils throughout the body of the prospective marine, linking the various synthetic organs and physiological mutations to the hulking power armor that the recruit will don when this last implementation matures. Most would be recruits don't survive the inhumanly painful and involved process. All the better; there is no room for weakness among the ranks of the god emperor's chosen Astartes

(If that's supossed to be service studs, initiate receiving the implants would never have one service stud, let alone three of them.)

The idea isn't that he's an initiate, you can just see his carapace showing through. That's why he looks a bit older. But yeah, they are service studs.
