Very nicely done. Boots need to be taller on Melissa's version, but you've covered most of the major incarnations here.
She's had too many looks to reasonably get them all. Only major omission is the new52 version. And if we're keeping to a balanced number, the mod 60-70s costume as well.
Note that she didn't debut with the headband in the 1983 version. The red shoulder pad costume debuted in issue 13. The headband was added several issues afterward. They tried the look for the 1984 movie, but ditched the headband and simplified the costume to create the 1984 version you see in the final film.
