Lady deathstrike, born yuriko oyama,was the daughter of a Japanese crime boss, scientist and former world war 2 kamakazi pilot known as lord dark wind. Feeling dishonored and resentful of the American occupation of Japan, he invented the adamantium bonding process to create super soldiers to destroy the Americans. Fortunately (and unfortunately) the plans and adamantium were stolen by the weapon x scientists and used on Logan. Falsely believing he stole it and willingly used it on himself, she implanted cybernetic attachments into her body and took up the name lady deathstrike.

No, that was Mariko Yashida, a woman who fell in love with him but was killed before they could marry.
She did appear in "X-2: X-Men United", though.

No, that was viper, also known as madame hydra. She actually blackmailed logan into marrying her in the comics. It was short-lived.
