Before Kyuubi. No one knows how but he had it before he left the hiding place for the civilians to find his parents during the attack.

yeah, thanks to that little flashback chappy we had a little while ago, we know it was at least before the kyuubi attack. makes me more curious as to how he got it! ><

I would assume before because he has it then and it's not a 'new' looking scar. That said we don't know for sure, hell we don't even know for sure that it IS a scar. It could be a birthmark of some kind.
