Spotty has a habit of feeling awkward or nervous around family members or friends that are in a sad mood so all that awkwardness causes him to basically make a little awkward smile without him even noticing.
But in this case, I guess it can also be some sort of method for comforting his lil' sister and letting her that he's there for her.
He isn't smiling because he sustains off of his sister's suffering. Trust me, Spotty can definitely be a piece of crap at times, but he isn't that much of a piece of crap.
