It was modeled in Blender and the model was then printed by a company called Shapeways. If you want some more info on how to do that, check out BlenderCookie.com. They have some great tutorials on how to set up a model for 3d printing. And very soon I'm going to do another tutorial for them showing how to set up a full color print like this one from Blender.
